According to an evaluation of the WHO, medication errors cause at least one death every day and injure approximately 1.3 million people annually in the US alone. Adverse drug events in US community hospitals cost more than 3,000 USD per incident on average and lead to a median increase of hospital length of stay of 3.1 days.
